I used to live and work in the UK. At the moment I am residing in Italy. Can I claim my tax and how?

What kind of documents would I need to send and who to in order to receive my tax back, if possible at all. Thank you!

If you send a P85 together with your latest P45 you should be fine.

I think you could claim tax back if you were resident for tax purposes in Italy whilst paying UK tax.

However you would have to have pre-registered as an italian tax payer first with the revenue

I dont think you can make any claim for repayment if the tax was paid prior to to that date. its possible that the revenue may be prepared to pay any allowance not used on PAYE. Supplying your P45, with an accompanying letter, to the tax office that used to handle your tax afffaris should trigger an appropriate payment
cliam it from the tax office where you have been paying your tax.
