LLC are issued by the state. It is an incorporation. Get a forms packet from your Sec. of State, fill it out, and send in the proper fees, and you should have you papers for the corporation.
apply at the place you got your license/ with the SBA.
you may not want to become an LLC but rather an S corporation. Talk to a lawyer about the differences and which one shields you better. I went with S corp years ago.

As a small business owner, I had asked the same question. I suggest getting a lawyer familiar with this type of business. They are more knowledgeable, and you can guarantee that you are following all procedures properly and more efficiently.
You need to get the forms from your state and then you will get the title, tax-id number you can use your social if you think it will be small. IRS.gov has a good booklet that you can download for help in starting your own small business.
