(I suggest you apply for a fresh PAN card, I am sure you know that it has to be applied through UTI bank branch.
In case you get the previous one also, get it returned to the IT deptt. immediately.
For surety, please talk to some CA and he shall guide you. Also you can visit the website of Income Tax India : incometaxindia.gov.in you shall get the details.

Just apply fresh online and submit hard copy at counter and then take persona name and tel no and do follow up on tel, otherwise it is never ending story
