Dear Mr. Tenant (use their name),

As the new owner of (their address) I want to take this opportunity to thank you for choosing to lease from me. Your new lease agreement is enclosed. As you will note, $xx.xx is due on the beginning of every month. Your payment may be mailed to (your address) or dropped in the lock-box located in (landmark).

Prompt payment is appreciated so that I may reserve your apartment for you. I have many applicants on a waiting list. If payment is not received by the 3rd of the month, I will have to assume you do not want to continue your lease, and will contact one of these applicants.

You will be notified of any changes to this monthly contract as they arise. Please call or come to my office if you have any questions regarding the new lease agreement. If you have no questions, simply complete, sign and return the renewal contract to me in the enclosed envelope.

Sincerely,

You can pick one up for really really reallly super cheap at an office supply store. Like for a buck fiddy.
I would seriously use a pre-made form. It is worth it, and it is essential to have carbon-copies.
Get one from your local public library.NOlo Press Books. Or buy a $29.99 Computer Program with all sorts of legal documents. Be smart;dont be cheap.
