This is identity theft and you should report it to the police and the credit bureaus. I know it sucks to have to go after your dad like that but a bankruptcy will haunt you. He broke the law when he got credit cards in your name. There is no reason for you to be the one stuck cleaning up the mess.
You don't need to file bankruptcy. You need to file fraud charges against your father. I know it's tough, but you MUST do this. He's ruining your life; you need to take it back and make HIM pay for what he has done.
You should be able to use IRS returns. The last 2-3 years should be sufficient, along with your most recent pay stubs.
Make copies, not the originals. Just have the originals in hand.

i dont think you need ALL your paystubs, just a few. you also need copies of your tax returns. i would file for an extension. if everything isnt perfect, youll have to go back anyway.
r u sure u have to file? it's hard to get homes and cars or if u have a home it's hard to refi. u should call like those get outta debt quick numbers i'm sure u can lower ur payments and put it all into 1 payment. u should look at all ur options first.
get a credit history report to make sure you know everything that is on your report and if it isn't yours you need to find out who made the bill.
