I would suggest you read the "definition of disability" contained in your policy. This will tell you whether or not you can work in another occupation and still be considered disabled. It will also tell you if you must be under a doctor's care.
Once you are on claim, you will receive periodic requests to take to your physician for updated medical information to continue the claim. The nature of the disability doesn't really matter.
If you will be filing for Social Security Disability, Worker's Comp or any other social insurance type benefit, the benefit you receive from your insurance policy could be affected. Many policies (especially group insurance policies) contain an integration clause (usually a dollar for dollar offset) that will reduce the insurance company's payment if you receive or are eligible to receive benefits from another source. Look for "Exclusions and Limitations" or "Benefit Integration" in your policy.
The insurer has the right to require occasional continuing proof of disability. They can request confirmation at future times. If you're on Social Security disability, that generally makes it easier.

When you are getting Permanent Disability you will upon demand from the WC carrier report on your condition. This report is also sent to your primary physician.
The Ins co will also negotiate with you on a settlement. This settlement after a period of time is for your benefit. As you know Ins companies not only pay benefits they also help those receiving the benefits get proper rehabilitation and care.
They want to see you "try" to make something of yourself for the rest of your life...rather than sitting around waiting for the check to come in the mail. They would like you to be rehabilitated and secure another job doing something that is not prohibited by your disability. They will help you. This doesn't mean you will not get any further benefits however... They will be adjusted according to your rehab and other employment.
