You're NOT going to have a lot of options. Assuming this is a new business, you'll have to find a local, independent agent who will most likely have to place this on the excess (surplus) market.

Rating will be based on your gross receipts, for GL coverage; and payroll for your workers comp. Both policies will be auditable.

I'd expect to pay $2500 for the GL, excluding liquor, and $1,000 minimum premium for the Workers comp. You'll probably end up with Scottsdale or Rockwood or Lexington or the like.
