What do UM and UIM rejection forms mean (in the personal auto policy)?

It means that this particular state allows you to reject uninsured motorist and underinsured motorist coverage. In order to do so you have to sign the designated forms. If you reject the coverage and have an accident that normally would be covered under those coverages you would not get to use those coverages. You also do not have to pay a premium for them due to the rejection of the coverage. Your choice if you want to reject.
Uninsured Motorist (should you have an accident with an uninsured motorist) and Underinsured Motorist (again, should you have an accident with an underinsured motorist). It is not a good idea to reject these coverages. Don't you want to protect you and your family to the same degree you are insuring youselves against a loss you might cause to someone else? There are a lot of "uninsured and under insured motorists" out there.
It means that you either do not wish to purchase uninsured and/or underinsured motorist coverage, or you wish to purchase it, but with limits lower than your liability coverage.
