Nothing can ever be pre-approved with workers comp. So go to the doctor, tell them you think it's work related, contact your boss to file the claim.
The longer it's been, the less likely you'll be able to collect. Especially if you haven't even seen the doctor yet.
Most states have a Workers Compensation Commission or Board. Check there first, but be aware that if you did not report your injury to your employer when it happened, you will have a harder time of it. Some states have very stringent guidelines for reporting.
i work in the medical office.. my first question for you is "was a claim filed with your employer".. if the answer is no.. that is step one. the employer must than contact their workers compensation insurance which in turn will issue your case a claim number. the claims adjuster and the case nurse manager will contact the appropriate doctors for your injury.. once a claim is established a claim can be filed with your local workers compensation department for wages during the time when you got injured
