Where do the CEOs and company presidents start their careers?

For large established businesses, a career in a Management consulting firms like McKinsey often lead to CEO and other executive jobs. For growing businesses, typically the CEO started as entrepreneurs with small businesses.
I think most start in the accounting departments of a company or in the sales or marketing department. The next logical step would be CFO and then finally on to CEO - a lot of CEO's were once a CFO and most have an MBA and started in middle management and also served on the Board of Directors. I am sure however that some have worked from the bottom of the company ladder all the way to the top, which is very impressive. A lot of the big companies will get a hired gun CEO from other companies but that CEO still started the way most other CEO's did at one time.
