All good answers so far. Also, ask at your bank. Just be sure to save all paperwork/receipts that come with it so that if something happens and it becomes demagnetised, you can go back to point of purchase and have it redone with the amount you still have on it. (This is why it's better to get it from your financial institution if they offer them.)
You have to go to a bank. They will charge you a fee (usually $5 for the card itself). Ridiculous, but they have to make their "cut" somewhere.
Do you have a Simon mall near your location if so you can get a Visa gift card. It has very few fees. It is not re-loadable.
Once you buy it. You can use your card online after you register it.
