If you owe money. you can't really cancel. Cut it up and look for more reasonable cards to transfer too. There are a lot of zero APR cards out there. do a search on-line. They may charge you for a transfer depending on your credit history, but you will save money in the long run.
Cut the card up and mail it back to them with a note to cancell your account immiedately and notify the credit agencies that you the customer closed the account.
Transfer the balance to a 0% card. Cut up the Permier Bank card and send it back to the with CANCEL.

You may want to consider keeping some of your cards open.
Pay them in full when possible and use them periodically to stay active.
Credit report shows your financial responsibility and is basically a "report card" on you.
If you've never used it and these are just the fees they've added then cut it up and send it to them along with a note, "certified", that way you get a receipt and you'll know exactly when they receive it back. If you've used it, unfortunately you have to pay what you've charged and what they already had on the card when they sent it to you because using the card is your acceptance of the fees. There are plenty of numbers for first premier.
