I assume that you are talking about a work visa like the H-1B visa, right? The Form with the bar code is might be from the electronically filed DS-156 form, but it is unlikely.

It could also be the Voucher # from the payment receipt for the processing fees. I would tip on the voucher that the fee were paid for you, because the DS 156 was probably not filled out for you. You will need that one though, so keep the link from above in mind or you will spend a lot of extra time when you go to get your Visa done.
To get the work visa issued into your passport you have to go to the nearest U.S. embassy or consulate in your country (find out the phone number and make an appointment before hand).
You will need the following (but ask them on the phone when you make the appointment, because you might need something else, which I not needed as German)
- Valid Passport
- Original Form I-797A (Case Type I-129)
Green form that confirms your visa application and that it was approved
- Form DS-156 Nonimmigrant Visa Application
A lot of personal data
- Form DS-157 Supplemental Visa Application
only for male applicants, asks about military background and stuff like that.
- 1 (bring 2) Photographs (37mm x 37 mm) (front-face)
- a receipt that shows that you paid the visa processing fee (around $100, I paid 80 Euros in March this year)
That's the round up. I would be surprised if your consulate is different than the one in Germany. NO electronic devices (including cell phones, iPods etc.), no bags, except a clear plastic bag with documents are allowed inside and there is no storage at the location to deposit it. Leave it at home or at a friend or in the car.
