Put the suggestions in there.
Unfortunately, an "effective force" usually dwindles when facing the big men/women, so be prepared to go it alone if it's a cause you feel fighting.

First document everything. Have hard data to back it up. If you haven't done that yet start. Then come up with a list of concerns. DON'T make it seem that you're out to get her or get emotional about it, give them the facts and only facts. Show them with documentation how it's effecting other departments or costing the Company money. After that the balls in their court, don't try to make the decision for them. They may decide that your CEO just needs some more training or is not be aware of the problems their causing.
If you do everything by the book, policy manual, and back everything up with paper work, then your risk of terminations is small. But the real worry is if they don't fire her how would that effect you.

I think board of directors are informed and a meeting is called. Evidence are presented and necessary action is taken.. Bord of directors are elected by stock holders of a company.
