Anyway, you don't say what the job is, so we really can't help you with this.
Personally I wouldnt answer it. I would request an interview, its a lytmus test of sorts.
If you have to answer it, know the person that reads it. You have to answer it how he/she would answer the question. You dont give your own answer you give his or hers. If the person that makes this decision is upset because the company didnt do this. Thats your answer for 1.
On number 2, you answer what you can do to help him answer 1.
And 3 you think the company isnt doing enough to help people in the company for the first answer that isnt if your issue.
There is your raise.
