This is very common with general appraisals. You need some clear measurement structures which define productivity in such a way that the results can be trusted and easily observed. The problem with subjective, non-metrical appraisal processes is that virtually everybody falls within a few degrees of everybody else.
Sit down and figure out what needs to be measured--output, quality, timeliness, for example. Then figure out how each of these can be measured. Your measurements don't need to be perfect, but they must be defensible and objective. Then, put together a program of actually performing the measurements and recording them on a daily, weekly, or monthly basis. Then begin this appraisal cycle with a clear definition to all employees of how this program is going to work. Set clear standards that are achievable but difficult. Then, measure and report to each employee frequently so there is no doubt when the annual appraisal is completed that it is fair and consistent with what the employee has been hearing all along.
