What is the protocol when getting promoted. Who to thank and how do you thank them?

There is no real protocol. if you want, ask how you were selected for the role. If you hear that other people put in a good word for you, take the time to visit with them personally and shake their hand. If your boss takes all the credit, tell him or her you will work hard to put them at ease with their decision and really appreciate the opportunity.
Gifts, cards, and overt gestures like that will be seen as butt-kissing. Don't go there.

You do not need to thank anybody because if you get the job, you should deserve it. In other words, the organisation could equally well thank you for taking it.
But it won't hurt if you mention to your boss that you are really pleased with the move.
