It is appropriate to send it at least 30 days in advance for preganancy, elective surgery etc, when you know that it is coming, but it is not required. You need to sign the letter, so an e-mail is okay, but you need to follow-up with the actual signed letter. Also, remember that e-mails get deleted by accidnet so keep a copy and best to hand deliver it or have it delivered registered mail. Keep the reciept.
