What's your hesitation about? Are you afraid they won't remember you, or be upset you reached out? They told YOU to contact THEM if you needed help, so definitely you should take advantage of that opportunity.
I would recommend calling, but think through exactly what you are going to say (or exactly what voice mail you are going to leave) before you call. You may want to write it down to be fully prepared.
Make it very simple, along the lines of:
"Mr. Smith, this is Tanya Jones at (999) 555-1212. When we spoke last December, you invited me to contact you if I ever wanted your help. I would love the opportunity to meet with you to discuss what I would like to do next in my career, and to get your advice. I am available for a meeting almost any day in the next 3 weeks, if that's convenient for you. I will follow up with you next Friday if I haven't heard from you in the meanwhile."
