No. Not if it's from an organization in the *private sector*.
For *government* organizations, that usually is the norm.

No, it's not necessary. The important things are that (a) it be on company letterhead and (b) that it be signed.
Not as far as I know. As long as the firm who gives it to you has their letterhead on it & it would be signed.
If its from an individual, you would probably just need to have all his contact details.

In India, letter head of compnay will do, but I have hear that in African countries, stamp if important, whithout that letter is not valid.
