When I was working on my MBA, we had a similar project, and I have always found that the best method is to use a short, concise survey with the potential of a reward for participating (i.e. random drawing for $100 per months, etc.)
You will want to use 10 to 15 questions, with very clear questions on what aspects of satisfacition that you are looking to measure (i.e. "This Bank has open hours that meet my schedule") you should also leave a blank space for customers that choose to leave more feedback.
Finally, the grading scale should also be simple..Dont use "Agree, Disagree, Strongly Agree, Somewhat Agree"..People hate that stuff. Give them a ten point scale, the higher the number, the more accurate a statement is.

to build on the last answer, all good, just wanted to address the number of questions, I like to use a few more questions, asking important questions twice, in different ways, to really getto the bottom of things. As for a prize, good idea, however instead of monthly, why not just one proze at the end of the research?
